What is melasma? Why does it occur?

Melasma is a skin condition characterized by dark or brownish patches on the face, caused by abnormal overproduction of melanin. The main triggering factors include:

Causes of Melasma

  • Sunlight (UV radiation) – the most important trigger, stimulating melanocytes to produce excess melanin.
  • Hormones – common in pregnant women (“pregnancy mask” or chloasma) or those using contraceptives/hormone replacement therapy.
  • Genetics – individuals with a family history of melasma have higher risk.
  • Skin inflammation – such as acne, aggressive exfoliation, or cosmetic allergies.
  • Environmental & lifestyle factors – heat, pollution, stress, lack of sleep.

Types of Melasma

Epidermal melasma

Dark brown, well-defined borders, responds well to treatment.

Dermal melasma

Grayish or bluish-brown, slower response to treatment.

Mixed type

The most common, a combination of both superficial and deep melasma.

Treatment Options

1. Prevention (most important)

  • Daily sunscreen SPF 30–50++ even indoors.
  • Avoid sunlight between 10:00–16:00.
  • Wear hats, sunglasses, or carry an umbrella.

2. Topical & Medical Therapy

  • Hydroquinone (HQ): inhibits melanin production (prescribed by doctors).
  • Tretinoin / Retinoids: accelerates skin cell turnover.
  • Topical corticosteroids: reduce inflammation (often combined in triple-combination creams).
  • Other agents: Tranexamic acid, Kojic acid, Azelaic acid, Arbutin, Vitamin C.

3. Clinical Procedures

  • Chemical peels: using AHAs or TCA to accelerate exfoliation.
  • Lasers & light-based treatments: Q-switched Nd:YAG, Picosecond laser, Fractional laser – must be done by specialists to avoid worsening pigmentation.
  • Microneedling with PRP / Polynucleotides: enhances skin regeneration and reduces chronic melasma.
  • Mesotherapy (skin booster): injecting vitamins or depigmenting agents directly into skin.

4. Adjunctive Therapies

  • Oral Tranexamic acid (under medical supervision).
